SUPPORT AGREEMENT

This Agreement (the “Agreement”) is made and entered into by and between Rachael Johnson and The Client.


1. Purpose

The purpose of this relationship is to support the Client in achieving their personal wellness goals, lifestyle, mindset, stress management, and overall well-being.

2. Scope of Services

  • Sessions: The Client will receive sessions lasting 50 minutes.
  • Session Format: Sessions will be conducted via virtual platform.
  • Ongoing Support: Limited support between sessions via email as outlined in the communication policy.
  • Not Provided: Rachael Johnson does not diagnose, treat, or cure medical conditions, nor provide therapy, medical nutrition therapy, or clinical interventions. Sessions are focused on wellness, self-awareness, and personal growth and are not a substitute for professional medical, psychological, or therapeutic care. Clients should consult a licensed healthcare provider for any medical concerns.

3. Confidentiality

  • Rachael Johnson agrees to keep all information shared during sessions confidential and will not disclose information to any third party without the Client’s written consent, except in cases where:
  • Required by law (e.g., court orders).
  • Belief the Client poses a risk of harm to themselves or others.

4. Fees and Payment

  • The coaching fee is $145 per 50 minute session.
  • Payment is due before each coaching session.
  • Accepted payment methods: [credit card, PayPal, bank transfer, etc.].
  • Late payments may result in a suspension of services.

5. Cancelling and Rescheduling Policy

  • Notice Requirement: The Client must provide at least 24 hours notice to cancel or reschedule a session.
  • Late Cancellation: Sessions canceled within 24 hours of the scheduled time may be subject to a cancellation fee of full session

6. Termination & Refund Policy

  • Either party may terminate this Agreement with written notice. Refunds will be prorated based on completed sessions.
  • This support relationship may be terminated immediately if:
    • The Client violates the payment policy
    • The Client engages in inappropriate or disrespectful behavior
    • The Client requires medical or clinical support beyond the scope of this support.

7. Disclaimer & Liability Waiver

  • Rachael Johnson is not a licensed medical professional, therapist, or registered dietitian. Coaching is intended for personal development and general wellness and is not a substitute for medical, psychological, or professional healthcare advice. The Client is responsible for seeking appropriate care for any medical or mental health concerns.
  • The Client understands that any dietary or lifestyle changes should be discussed with their physician, dietitian, or healthcare provider before implementation.
  • Rachael Johnson is not liable for any adverse effects, health complications, or lack of results arising from coaching services.
  • The Client assumes full responsibility for their choices, dietary changes, and personal health outcomes.

8. Governing Law

This Agreement shall be governed by and interpreted in accordance with the laws of Colorado, USA, without regard to its conflict of law principles.

9. Entire Agreement

This Agreement represents the entire understanding between the Coach and Client. It supersedes all prior agreements, discussions, and understandings regarding the coaching relationship. Any modifications to this Agreement must be made in writing and signed by both parties.

COMMUNICATION AGREEMENT

Overview

Clear and effective communication is essential for a successful relationship. This policy outlines expectations regarding response times, communication methods, and boundaries to ensure a smooth and professional experience.

Approved Communication Methods:

To maintain consistency and professionalism, coaching-related communication should take place through the following methods:

  • Email: Rachael@thrivingtype1.com – For general questions, and non-urgent matters.
  • Video Calls: Conducted via virtual platform and scheduled in advance.

Response Time:

  • Emails will be responded to within 1 week.

Availability:

  • Sessions are available during our mutually agreed upon time.
  • If changes to availability occur, clients will be notified in advance.

Emergency Situations:

This support is not a substitute for professional medical, legal, or crisis support.

In case of an emergency, please seek immediate assistance from the appropriate professional or emergency services.

Boundaries & Respect:

Respect for personal time is essential. Unscheduled calls or messages outside of agreed-upon hours may not receive a response.

Messages or inquiries should remain professional and relevant to the coaching relationship.

Rachael Johnson reserves the right to address any concerns regarding excessive or inappropriate communication.

Changes & Amendments:

This policy may be updated as needed.
